How to register
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
We require that when you register with us, you book an appointment with the practice nurse for a New Patient Screening.
You can register by completing the on-line registration form (GMS1) below, or collecting one from reception. If you need help filling out the form, please let a member of staff know.
Appointments
We run all our surgeries by an appointment only system. We operate the Advanced Access Appointment system, this means appointments can be booked by ringing (01690) 710205 from 8:00am on the day you wish to be seen. We can also pre-book the first two appointments and the last appointment of the day with each doctor.
If you are too ill to come to the surgery and need a visit at home, please call the surgery before 10am. Home visits take up much more of a doctor’s time than a consultation in the surgery, so if you are mobile please come into the surgery to be seen.

Repeat prescriptions
- You can order your repeat prescription at reception, by fax, by posting it to the surgery or online.
- Tick the items you need on the right-hand section of your previous prescription form.
- Your prescription will be ready for collection a minimum of 2 full working days later (our working days are Monday - Friday, 8:00am - 6:00pm).
We do NOT accept repeat prescription orders over the telephone.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
It is the policy of the Conwy Local Health Board that we can only issue prescriptions for one month at a time.
Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Test results
Since our telephones are very busy throughout the day, we ask you to telephone the nurse for test results (eg blood, urine, x-ray) between 1.00 and 2.00 on Mondays, Tuesdays, Thursdays, and Fridays.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery.
